Requirements
- Application form from the Immigration office
- 2 Passport photos
- Written consent letter form parents or legal guardians
Process
- Applicants must complete and sign the passport application form. Original or certified copies of appropriate supporting documents must be enclosed (Refer Section D – Checklist in application form).
- Application of children under 16 years of age:
a. Complete the consent form on Section C of the application form.
b. Written consent from either parents or legal guardians is required. - Lodge your application at: The Immigration Office, Level 1 Government Building, Eleele Fou, Samoa
- Lodge two photos. The person who certifies your identity in Part B of the form must endorse the back of one photo: “This is a true likeness of (your name)”
